Please fill out and submit your enrollment form below before purchasing your program of choice. We will ask you to confirm information previously submitted so that we may link your reservation with your enrollment. Once the enrollment form has been filled out and submitted, please click on shop in either the individual submissions or multiple submissions w/discount. Once your program is purchased we will link it to your enrollment form and you will receive an email from us with detailed instructions on sending us your first video submission. We want to take this time the thank you again for participating in our program.

Acceptance Agreement and Release of Liability Waiver: Once the Enrollment Application is complete and submitted, I will choose the corresponding program from the shop and check out. I understand there are no refunds for any reason, at any time once enrolled, as this is an educational video correspondence program. I understand that if I am not satisfied with the educational feedback I have received I can reach out to the program director to see what my options are. I understand horses, by nature, carry inherent risks.

By signing this agreement, I understand that I (and my minor) are giving up certain legal rights, including (but not limited to) the right to sue, or recover damages in the case of loss, property damage, injury or death for any reason, including but not limited to the negligence of the Instructor(s) and/or their agent(s) while enrolled in Mars | Venus Equi-Pro LLC's Long Distance Equine Education Program.

I acknowledge that horseback riding is an inherently dangerous activity and horses are an inherently dangerous animal, and that without warning or without any apparent cause or reason they can stumble, trip, roll, fall, buck, rear, kick, bite, run, make unpredictable movements, spook, jump obstacles, step on toes or feet, push or shove a person, saddles and/or bridles can loosen and/or break, all of which can cause the rider/spectator to fall or be jolted, that may result in serious injury and in some cases death. I voluntarily assume all risk(s) and danger(s) of injury or death associated with the handling or riding of ta horse, and use of saddles, bridles, halters, equipment and gear while participating in this long distance equine education program.

I give up all rights (both mine and those acting in my behalf) to sue or recover compensation of any kind from Casondra Goldman, “Mars | Venus Equi-Pro LLC”, its employees, agents, or other instructor(s) for any loss, injury (including, but not limited to, death), damage, or cost to me, my child, or my property while participating in this long distance equine education program.

I understand it is my duty to uphold this agreement in its entirety and I agree with all that is herein. This is a legal agreement that can and will be upheld in a Court of the Law.

I have read and understand the Privacy Policy and Terms and Conditions found on the FAQ page at https://www.mvequipro.com/faq.html and agree to accept the Terms and Conditions, Privacy Policy and this agreement known as the Acceptance Agreement and Release of Liability Waiver by continuing with my enrollment.

I have concluded that the release and liability waiver is worth the pleasure of participating in this program. I understand that photos and/or videos submitted should not go against any copyright infringements or laws. I agree to and grant permission for the photo or video to be used on different websites such as Facebook, Twitter, Instagram and other social media, as well as www.mvequipro.com, if not otherwise indicated on my enrollment application.

By submitting this enrollment application I am accepting and agreeing to the terms above and committing to my enrollment.
