Recurring Payment Authorization

By enrolling in recurring payments, you agree to the following:

I authorize Mars | Venus Equi-Pro LLC to automatically charge my payment method on file for recurring lesson or service fees according to the selected payment plan. I understand that:

• Payments will be automatically charged to your card on file on a monthly basis.

• Payments are due before instruction is given. To maintain your scheduled time slot and continuous enrollment, please ensure payment is made by the last lesson of each current package or by the 1st of each month.

• No refunds will be issued at any time, for any reason.

• A 2.9% card processing fee will be applied to all credit/debit transactions.

• If you’re starting mid-month, your first payment may be prorated accordingly.

• Returned payments, declined charges, or canceled transactions will incur a $45 fee. That payment method will no longer be accepted for future transactions.

• Missed sessions without a minimum of 24-hour notice will be forfeited and charged in full.

• This authorization remains in effect until you notify us in writing of cancellation.

• Wave is used for pre-authorization requests and payments will be completed through Wave as our payment processor.

By signing up for recurring payments, you confirm that you have read and agree to our full payment policy and have completed and signed the required Registration & Release of Liability Form.
